The Anatomy of the Breast

The breast tissue consists primarily of connective tissue, fatty tissue and glandular tissue, determining the breast’s size and shape. It is richly supplied with blood and lymphatic vessels, offering protection against infections. Milk-producing glands are surrounded by fatty tissue and ligaments. The firmness of the breast depends on how well these ligaments adhere to the chest wall. Unlike other parts of the body, the breast tissue does not contain muscle tissue (except for tiny muscle fibers in the nipples); instead, there are two chest muscles underneath (pectoralis major and pectoralis minor) that separate the breast from the chest wall.

Aging, pregnancy, and breastfeeding can cause the skin and tissues of the breast to lose their elasticity, altering the breast’s size and shape. These changes can be aesthetically and emotionally challenging.
Implants can be placed between the glandular tissue and muscles, entirely beneath the muscles and connective tissues, or partially beneath the muscle. The choice depends on your specific needs and our surgeon’s recommendation. Neither of these procedures will affect the possibility of future breastfeeding or the performance of diagnostic tests (CT, MRI, ultrasound, mammography).
However, severely sagging breasts cannot be restored solely through implants. In such cases, breast lift surgery (mastopexy) is required in conjunction with implant placement. Both procedures can be performed in one surgery session.

Types of Implants:
Understanding Your Options

Breast implants come in various shapes, textures, and projections. They can be round, oval, or teardrop-shaped. The surface can be smooth or textured, and the projection can be standard, moderate, or high profile. Sizes also vary, leading to differences in overall dimensions. We always work with reliable, high-quality breast implants that come with a lifetime guarantee from the manufacturer and have the mandatory FDA and CE certification in the European Union. These implants are filled with cohesive silicone gel and have a textured surface (micro- or nanotextured) to promote secure tissue adherence. The selection of implants is an important and responsible task, and we choose them collaboratively with the patient during consultation, carefully considering physical characteristics, patient preferences, and the placement location—whether under the muscle, partially under the muscle, or under the glandular tissue. Patients receive a warranty card for the implanted implants, detailing the implantation date, type, size, and manufacturing serial number. The implant we use in our practice: POLYTECH, MOTIVA, and MENTOR.

Silicone implants

Silicone implants are filled with medical-grade silicone gel, mimicking the feel of natural breast tissue. If a silicone implant ruptures, the gel may stay within the implant shell or leak into the surrounding area. Regular check-ups are necessary for silicone implants. Implant surfaces can be smooth, micro-textured, macro-textured, or covered with a layer of Microthane (micropolyurethane foam). Implants with a Microthane coating or textured surface have the lowest chance of developing capsular contracture (thickening of the collagen fibers around the implant).

Polytech implants

Polytech Health & Aesthetics breast implants contain the highest quality cross-linked silicone gel. Thanks to the gel’s consistency, the implant maintains its shape even if cut in half, as it has a “memory” that allows it to return to its original form after any pressure or deformation.

The elastic, chemical, and mechanically resistant implant shell consists of various layers of silicon elastomer with predefined thickness. To ensure this high quality, the shells surrounding the silicone gel-filled implant have a special protective layer to prevent leakage of low molecular weight silicone components.

The surface of the implants can be smooth, textured, or covered with a Microthane (micropolyurethane foam) layer. Implants with Microthane covering and POLYtxt (textured) implants have demonstrated the lowest likelihood of developing capsular contracture.

Motiva implants

Each Motiva® implant is manufactured with the patented SmoothSilk®/SilkSurface® design. This is a smooth surface created with uniform, perfectly controlled nanotechnology across the entire surface of the implant. Based on test results and market monitoring, we can state that the surface:

  • Minimizes inflammation and ensures maximum biocompatibility (reducing the chance of bacterial adhesion and biofilm formation).
  • Allows for the insertion of the implant through a smaller incision (thus reducing related risks) and reduces the risk of double capsule and late seroma. Motiva implants do not have the risk factors associated with macrotextured surfaces, as the SilkSurface® is categorized as a smooth surface according to the latest international standard (ISO-14607:2018).

Some safety features of Motiva implants include:

  • BluSeal®: This patented visual safety indicator gives Motiva implants (excluding sizers) a bluish base color, allowing surgeons to check before implantation that the shell is completely intact. The colored sealing layer in the manufacturing process provides a significant advantage, as any material defect can be immediately detected, preventing defective products from leaving the factory.

Mentor implants

Founded in 1969, Mentor®, with decades of professional experience, is one of the world’s leading manufacturers of breast implants. In 1995, Mentor® became the first company producing breast implants to earn the CE mark for its products. In 2006, it also obtained approval from the Food and Drug Administration (FDA). The FDA is considered the strictest quality assurance authority globally, overseeing every phase of the manufacturing process.

Mentor® implants filled with gel are referred to as Memory Gel™. In Memory Gel™ breast implants, there is a uniquely cohesive gel with varying levels of cohesiveness. Cohesiveness refers to the material’s density, compactness, and ability to stick together. The silicone gel from Mentor® is a cohesive polymer, not a liquid. Although it feels soft to the touch, it behaves as a unified whole, resembling traditional breast tissue. This is crucial because no matter how the implant is compressed, it always regains its shape. They are called Memory Gel™ because the gel essentially has a “memory,” remembering the shape it should have. Implants filled with this gel typically result in a more natural feel and appearance.

Smooth Surface Implants

These implants feel the most natural. They move along with the natural tissue and capsule, appearing more natural during body movements. However, in some cases, visible or palpable rippling can occur under the skin.

Textured Surface Implants

Textured implants promote the formation of scar tissue around the implant, limiting the movement of the implant. Textured implants were less likely to cause capsular contracture.

Surgery Duration and Safety

Breast augmentation surgery, routinely performed under general anesthesia for patient tranquility and safety, typically takes 1-2 hours and necessitates a 1-night stay at the clinic for post-operative observation. It’s crucial to note that breast implants are not permanent solutions and may require future replacement due to changes from pregnancy, weight loss, or menopause, necessitating regular check-ups. We predominantly employ the inframammary incision technique, favored for its precise implant placement and minimal tissue trauma. This common technique involves a 4-5 centimeter incision in the breast fold (inframammary fold), offering excellent visibility for our surgeon. It ensures meticulous procedure control, minimal breast tissue manipulation, and avoids disrupting milk ducts and nerves, thereby reducing the risk of nipple sensitivity alterations and breastfeeding issues. Patients generally benefit from quicker healing and less discomfort due to the reduced tissue trauma.

Inframammary Incision

This is the most common technique where the incision is made in the fold under the breast (inframammary fold). The length of the incision varies based on the size of the implant and the individual’s anatomy but is typically around 4-5 centimeters. The inframammary incision provides good visibility for the surgeon, allowing for precise placement of the implant and meticulous control over the procedure. This technique involves minimal manipulation of breast tissue and avoids interference with milk ducts and nerves. As a result, there is a lower chance of nipple sensitivity changes and breastfeeding difficulties. Due to minimal tissue trauma, patients often experience faster healing and less discomfort following surgery.

Preoperative Steps

Before the surgery, you will:

  • Have a complete blood count and an EKG.
  • Under 40 years old an ultrasound of the beast is necessary
  • Older than 40 years old women need mammography and ultrasound
  • Adjust or stop certain medications.
  • Quit smoking.
  • Avoid taking Aspirin or specific anti-inflammatory drugs.
  • Refrain from using recreational drugs like cocaine.

Understanding Risks and Complications

It’s crucial for you to thoroughly discuss these risks with our plastic  surgeon, understanding the potential challenges before making an informed decision about breast augmentation. We will provide detailed information about minimizing these risks and ensuring a safe and successful procedure. Remember, while complications can occur, many patients experience positive outcomes when proper precautions are taken.

Common complications include:

  • Thick, Noticeable Scarring: Sometimes, scars may remain prominent and not fade over time.
  • Capsular Contracture: Hardening of breast tissue due to the formation of scar tissue around the implant.
  • Implant Rupture: Leakage from the implant, leading to the formation of small lumps known as silicone granulomas.
  • Implant Wrinkling: Visible creases or folds in the implant, especially if there’s minimal tissue coverage.
  • Implant Rotation: The implant might rotate within the breast, altering its position.
  • Implant Rippling: Rippling can occur if the implant’s surface sticks to a thin layer of tissue, causing noticeable irregularities.
  • Infection: Infections can arise, often necessitating implant removal.
  • Breastfeeding Issues: Some individuals might experience difficulty breastfeeding or produce less breast milk than before.
  • Dissatisfaction and Reoperation: Patients might not be satisfied with the results, requiring additional surgeries for corrections.
  • Nerve Damage: Nerve damage can lead to altered nipple sensitivity, ranging from heightened sensitivity to loss of feeling. While it might be temporary for some, it could be permanent for others.

Less common complications include:

  • Excessive Bleeding: Occurring during the surgery.
  • Allergic Reactions: Reaction to anesthesia.
  • Deep Vein Thrombosis: Formation of blood clots in deep veins of the body

Recovery Period

After the surgery, your breasts will be bandaged, and you’ll wear a sports bra with a strap. You will spend one night at the clinic after the procedure. Drains (tubes for fluid drainage) will be inserted into the breasts and removed the next morning by our surgeon. Pain relief and antibiotics will be prescribed for infection prevention.
You’ll wear a sports bra for 8-12 weeks. Avoid push-up bras or underwired bras during the entire healing period. A compressive strap must be worn both day and night for 2 weeks. We recommend 2 weeks of rest and gentle care at home. Physical activity should be minimized, including avoiding heavy lifting, active movement, and driving. After 2 weeks, lighter physical work or office work are permissible. It’s crucial to follow our instructions precisely. Follow-up examinations are scheduled at 1, 2, 3, weeks post-surgery. During the 3rd-week follow-up, the stitches will be removed, significantly easing our patients’ daily lives. Silicone tape application is recommended for scar management. For a few months, it is advisable to avoid intensive sports, direct sunlight (use sun protection!), sauna, and tanning beds.
Following the procedure, patients may experience tightening pain, a slight increase in temperature, and mild itching at the incision site, all of which are entirely natural. If you experience any unusual symptoms, please contact us immediately!
HYGIENE: Until the stitches are removed, the incision should not come into contact with water. After the stitch removal, short showers with running water are allowed, but bathing and soaking should still be avoided. After showering, gently pat the surgical area dry with a clean, soft towel—never rub or use harsh movements! After the shower, apply a healing gel or patch to the surgical area, and immediately put on the compression bra.

Post-Surgery Expectations

From the moment your surgery is completed, you’ll embark on a transformative journey. Initially, you’ll observe an immediate change in your appearance, with your breasts taking on a fuller, more aesthetically pleasing shape. As the weeks progress, the true beauty of your results will gradually unveil. Swelling will subside, and your breasts will soften, settling into their new, luscious form.
